Cancellation Form

We understand that life happens and that sometimes you need to take a break from classes.

We use an outside billing system and our monthly fees are submitted in advance of the first of the month. Please note that per our policies and procedures agreement that you signed when you enrolled, WE REQUIRE TEN (10) DAYS NOTICE TO PAUSE YOUR CLASSES.

- This means that if you are canceling after the 20th of the month, you will be billed for the next billing cycle.

Please also communicate if you are withdrawing from class immediately OR if you will finish out the month you paid for.

By signing this cancelation form, I understand the terms & conditions of pausing and stopping classes. If you stop after the 20th of the month, I understand that I will be billed for the next month cycle.
