SCY - Credit Card Authorization Form (Feb 2021)

This is the credit card authorization form to register for Shawn-Caulin Young's online zoom acting classes. Please know that Shawn's classes often have a waitlist to join. If the class you are interested in is full, please contact Diane Marie at the studio to be added to the waitlist. The studio number is 561-840-5030.

Please know that once you enroll, we hold your seat each month. If you decide you want to stop classes, you can do so at any time, BUT you MUST provide 10 Days written notice prior to the next month's billing.

Shawn is also available for private coaching. His rate is $125 per 30-Minute session and $180 per 60-Minute session.

Please know that we hold a seat for you in the primary class in which you are registered. If you cannot make a class, please text us to let us know, so we can advise the instructor. We understand that life happens... most studios do not allow talent to make up a class. At the Green Room, we allow you to make up one (1) class in a month as long as you notify us in advance.
